چکیده

INTRODUCTION Short-stemmed hip implants have been introduced in order to conserve proximal bone mass and could facilitate the use of minimally invasive surgery (MIS), in which smaller incisions limit access to the hip joint. MIS techniques have been associated with faster rehabilitation, reduced peri-operative blood loss and less post-operative pain [1]. The limited access during MIS may increase the risk of surgical mal-placement, the influence of which on the internal loading of the proximal femur is unknown. While conventional stems have been extensively studied in terms of the influence of design, surgical and patient parameters on femoral loading, little is known about the influence of such parameters in the case of short-stemmed implants. Changes to the internal loading of the proximal femur have been associated with bone resorption [2], which may lead to implant loosening [3]. The goal of this study was to examine the influence of surgical implantation, as measured by effective offset and anteversion, of a short-stemmed implant on the strain energy density within the proximal femur. 0 10 20 30 5 6 7

Bone density study of the proximal femur after hip arthroplasty with porous-coated implants

Progressive loss of bone mineral density around the femora! component of total hip replacement continues to pose a threat to long term prosthetic survival. A linear study was undertaken to measure bone mineral density on a monthly basis following total hip arthroplasty in 11 male patients. The opposite femur was used as the control measurement. Bone mineral density was unchanged at two mo...

Effects of stemmed and nonstemmed hip replacement on stress distribution of proximal femur and implant

BACKGROUND Despite improvements in shape, material, and coating for hip stem, both stress shielding and aseptic loosening have been the major drawbacks of stemmed hip arthroplasty. Some nonstemmed systems were developed to avoid rasping off the intramedullary canal and evacuating the bone marrow due to stem insertion. METHODS In this study, the finite-element models of one intact, one stemmed...

Mapping the strain distribution on the proximal femur with titanium and flexible-stemmed implants using digital image correlation.

We implanted titanium and carbon fibre-reinforced plastic (CFRP) femoral prostheses of the same dimensions into five prosthetic femora. An abductor jig was attached and a 1 kN load applied. This was repeated with five control femora. Digital image correlation was used to give a detailed two-dimensional strain map of the medial cortex of the proximal femur. Both implants caused stress shielding ...

Biomechanical optimization of the angle and position for surgical implantation of a straight short stem hip implant.

Conservative hip implants preserve healthy bone for revision surgeries and improve physiological loading; however, they have little supporting biomechanical data with respect to their 3D orientation during implantation. This study endeavored to determine the optimal 3D orientation of a straight short stem hip implant within the proximal femur that would yield a stress distribution most similar ...

Changes in Strain Patterns After Implantation of a Short Stem With Metaphyseal Anchorage Compared to a Standard Stem: An Experimental Study in Synthetic Bone

Short stem hip arthroplasties with predominantly metaphyseal fixation, such as the METHA® stem (Aesculap, Tuttlingen, Germany), are recommended because they are presumed to allow a more physiologic load transfer and thus a reduction of stress-shielding. However, the hypothesized metaphyseal anchorage associated with the aforementioned benefits still needs to be verified. Therefore, the METHA sh...
